Support the team with medication related queries. Routinely analyse current patient treatments and evaluate medication use. Educate patients on medication use and side effects to help support their treatment plan. Enrolled on the Primary Care Pharmacist Education Pathway.

What I can do

• Processing discharge summaries and letters
• Advice on alternatives to medication out of supply
• Working with GPs to devise personalised treatment plans
• Patient education: Consultations with patients To explain medication, side effects, use and storage of medicines, General lifestyle advice and support.
• Medication reviews and Long term condition reviews (LTC part2) for hypertension patients
• Statin initiation and education
• Reauthorising medication
